FRMP Respite Funding for Carers Application Form Embedded

Funds or grocery vouchers are available to assist with the living costs associated with a young person’s short-term respite with a carer (e.g. extended family members or family friends), while family reunification or other housing options are explored. Please note that in addition to applications for respite funding, support workers are encouraged to consider submitting an application for Therapeutic Interventions on the young person’s behalf. What FRMP will not fund:

• Emergency accommodation
• Unsafe and inappropriate accommodation

FRMP brokerage is allocated to assist the young person to:

• stay or return home, if safe and appropriate,
• connect/reconnect with family, significant others or community,
• recover from recent family conflict and breakdown
• build on their capacity to create 'families of choice'

By submitting this application:

• I confirm that I have obtained consent for the applicant to provide their personal information to MCM as part of the FRMP application process and I can evidence this consent.
• I also confirm that I have informed the applicant that their personal information will be recorded electronically in MCM’s systems such as our case management and finance systems.
