FRMP Therapeutic Interventions Application Form Embedded

These funds are available for:

• Individual therapeutic support (counselling, psychotherapy and alternative therapies such as art therapy, music therapy, equine therapy)
• Family therapy
• Family mediation

These interventions are provided by FRMP approved practitioners including, psychologists, counsellors, family therapists, family mediators, mental health social workers and alternative therapists.

Funding is allocated to assist the young person to:

• stay or return home, if safe and appropriate,
• connect/reconnect with family, significant others or community,
• recover from recent family conflict and breakdown
• build on their capacity to create 'families of choice'

FRMP brokerage is allocated to assist the young person to:

By submitting this application:

• I confirm that I have obtained consent for the applicant to provide their personal information to MCM as part of the FRMP application process and I can evidence this consent.
• I also confirm that I have informed the applicant that their personal information will be recorded electronically in MCM’s systems such as our case management and finance systems.
